What Is Breast Reduction?

Breast reduction, also referred to as reduction mammaplasty, is a procedure that removes excess breast fat, glandular tissue, and skin to achieve a breast size more in proportion with a woman’s body. The main goal of this surgery is to create lighter, firmer, and more lifted breasts that are more comfortable to live with. Breast reduction typically takes between 2-5 hours to perform under general anesthesia as an outpatient procedure. On average, this plastic surgery reduces each breast by about one pound of excess tissue.

Inverted-T Skin Resection

This common technique is also known as Wise-pattern skin resection. It involves making an incision around the areola, vertically down to the breast fold, and horizontally along that fold. This pattern resembles an anchor or an inverted T. It allows the surgeon to remove a lot of breast tissue, fat, and skin. In addition, the nipple-areola complex is repositioned higher on the breast mound. This technique produces visible scarring in an anchor shape on the breast but creates aesthetically pleasing contours.

Inferior Pedicle

Using this approach, the surgeon preserves blood flow and nerve supply to the nipple-areola complex by leaving an intact tissue pedicle in the lower breast region. Through an anchor incision, the doctor removes excess tissue from the upper, medial, and lateral areas of the breast. Then, the pedicle is repositioned upwards, and the skin is redraped to lift and reshape the breast. This technique can reduce breast size while allowing future breastfeeding by preserving nerves and milk ducts.

After confirming that the patient is eligible for breast reduction, Dr. Bailey provides preoperative instructions on medications and products to avoid before surgery that can increase bleeding risks.

One day before the scheduled operation, the patient comes to our office to have markings made on their breasts. Following a detailed plan mapped out from the measurements and photographs taken during the first meeting, Dr. Bailey draws lines on the skin that show how the breast tissue, nipple, and areola will be repositioned and corrected. These body markings are an important preparation step that guides the precise cuts, reshaping, and sutures during the procedure.

Breast Reduction Procedure

General Anesthesia

Dr. Bailey performs breast reduction surgery under general anesthesia. The patient is given medication intravenously to put them into an unconscious state before the operation begins. This ensures that the patient feels no pain or discomfort during the procedure.
Dr. Bailey makes incisions to start reshaping the breasts. The size and shape of these cuts depend on the technique, the amount of tissue that needs to be removed, and the desired breast shape. In very rare cases when mild reduction is needed, the surgeon can create a shorter incision without a horizontal cut along the breast crease.
Dr. Bailey removes skin, breast tissue, and fat to reduce the breasts to a size that is more proportional to the patient’s body. This is done using a scalpel, surgical scissors, and other instruments.
The remaining breast tissue is reshaped, and the nipple-areola complex (NAC) is repositioned to a higher level. The NAC remains attached to nerves and blood vessels underneath to maintain sensation and viability. Once the breast is reshaped to fit the new contour, the remaining excess skin is trimmed away.
Internal incisions are closed with layered sutures in the breast tissue. Dr. Bailey also uses stitches, glue, tape, or bandages to seal the external incisions. Temporary drainage tubes may be placed to reduce post-operative swelling and promote healing.

Recovery After Breast Reduction Surgery

Recovery from breast reduction surgery typically takes several weeks. Patients wear surgical bras during the first 7 days and have drainage tubes to remove fluid and prevent infection. The initial recovery period involves managing pain and discomfort with medication. Patients are advised to avoid strenuous activity for 4-6 weeks to allow incisions to heal. Most people can return to work within 10-14 days if the job does not involve heavy lifting or physical exertion.

Full recovery takes about 3-6 months. During this time, swelling gradually subsides and sensation in the breasts returns. Scars from the procedure continue fading over several months. Patients might feel some changes in nipple sensitivity, which could last up to a year.
