What Is a Spider Vein?

The medical term for spider veins is telangiectasias. These small red and purple veins are approximately 1-3 millimeters in size and can occur anywhere on the body. Patients most commonly seek treatment for spider veins on the legs, face, and chest. They are often found in combination with varicose veins. There are no health risks with spider veins, but they are a common cosmetic concern. The tendency to develop this condition is inherited but can also occur after localized trauma and may worsen with pregnancy.

Causes of Spider Veins

Hormonal changes, especially in women, contribute to the development of spider veins. Fluctuations in estrogen and progesterone may cause veins to widen and become more visible. Pregnancy, menopause, and birth control use are usually associated with hormone shifts that might lead to this condition.
Individual genetics play a significant role in creating spider veins. If the patient’s parents or other family members have prominent leg veins, they are also at a greater risk for developing them. Defective valves or weak vein walls can cause veins to bulge and twist.
Exposure to ultraviolet rays from the sun may affect the veins over time and make them appear twisted and swollen on the skin’s surface. This cosmetic problem can occur after years of sunbathing or incidental exposure.
Trauma or injury may also contribute to spider veins. Anything that damages the vein valves, like a bruise, fracture, or surgery, can restrict proper blood flow. The increased pressure causes veins near the surface to expand into spindly spider vein clusters.
As we age, our veins lose elasticity, and the valves that prevent backward blood flow weaken. This causes blood to pool in surface veins, stretching them and making them more visible. Years of wear and tear lead to more spider veins in older individuals. They commonly occur when people turn 40 and become more prominent with age.

Spider Vein Treatments

Our spider vein treatments include sclerotherapy, surface laser, and intense pulsed light (IPL) therapy. Some patients require a combination of these procedures for the best results. Here is an overview of each treatment:

Visual Sclerotherapy

Sclerotherapy is a minimally invasive procedure that involves injecting a solution into varicose veins to cause them to collapse and fade. Dr. Bailey uses one of two FDA-approved medications (Sotradecol® and Polidocanol®) for sclerotherapy. The cosmetic results are much better, and there is less pain than traditional hypertonic saline injections. The solution is injected into the spider vein through a micro-needle, causing it to seal shut. Blood then flows into healthier, deeper veins. Eventually, the problem disappears completely.

IPL Therapy

IPL therapy involves broad-spectrum light to target hemoglobin in the blood within abnormal veins. We use the Palomar system, which produces light pulses that the vein walls absorb, causing them to heat up, collapse, and seal shut. The veins continue to close over multiple sessions and eventually disappear. IPL works well on small facial spider veins and tiny formations on the legs. It can also improve overall skin tone and texture.

Surface Laser Treatment

Surface laser treatment is accomplished using the Iridex 532 and Sciton 1064 YAG devices. These systems produce specific wavelengths that seal the spider veins. It is the preferred treatment for face and chest veins.

Recovery After Spider Vein Treatments

There is often a brief recovery period after receiving treatments for spider veins. Following sclerotherapy, where a solution is injected into the veins, patients may experience mild discomfort, bruising, swelling, and redness lasting 1-2 weeks. They may be required to wear compression stockings after this procedure to aid healing.

Laser treatments and IPL therapy usually involve less downtime. These non-invasive procedures use light energy to improve the appearance of veins just under the skin’s surface. Mild redness and swelling may persist for 12-24 hours. Most patients can resume normal activities immediately after laser or IPL treatments for spider veins. Although individual experiences vary, modern therapies require little recovery time yet yield significant improvements.

Benefits of Spider Vein Treatment

Most patients seek spider vein treatment due to cosmetic concerns. The affected veins often create an unsightly appearance and make people self-conscious. They can also indicate vascular insufficiency, eventually causing more complex conditions. The benefits of undergoing spider vein treatment include:

Sclerotherapy and laser procedures reduce the visibility of unsightly veins, improving the aesthetic appearance of the legs.
Spider veins are sometimes associated with aching, burning, swelling, or night cramps. Treating them provides relief from uncomfortable symptoms.
Diminishing the look of spider veins can help boost confidence and self-esteem.
Modern spider vein treatments do not require much downtime or disruption to daily life. The procedures are performed comfortably at our office.

Frequently Asked Questions

A. Varicose veins are enlarged, twisted veins that usually occur on the legs and ankles. They are often blue or dark purple and tend to bulge out from under the skin. These veins develop when their internal valves stop working correctly, allowing blood to pool and the veins to stretch and twist. This may lead to blood clots, ulcers, bleeding, inflammation, and pain. Dr. Bailey can treat varicose veins with endovenous radiofrequency ablation, microphlebectomy, and other advanced procedures.
A. Reticular veins are often seen as blue-green formations under the skin. They can become more prominent and lead to the development of spider veins. Some patients may experience tenderness, burning, and itching in the legs.
A. Venous ulcers or stasis ulcers are often the result of long-standing vascular insufficiency (faulty vein valves or reflux). In most cases, the reflux occurs in the saphenous and perforating veins. The high pressure within these veins eventually leads to chronic leg swelling, darkening of the skin, and ultimately, skin breakdown, ulceration, and even bleeding. The ulcers are generally near the ankle and are very painful.
Dr. Bailey is one of the first 11 physicians in the country to use the revolutionary VNUS radiofrequency perforator closure system and has taught physicians from around the Midwest how to utilize the device.
